Lungswell
The chest tightens and grows heavy with fluid; a too-common end. Every day spent in proximity to an infected person, Save vs Poison or become infected: Lose 2 Constitution and lose 8 Poison Save. Additionally, infected people cannot recover HP or wounds by resting. Every month, Save vs Poison to recover, or else take 1d8 damage.
Bone Char
Inner organs blacken and bone singes with incessant pain like hot coals. Caused by motes that live alongside smoldering salamanders, among others. Every month, or when taking damage from heat, randomly lose 1 point from Strength, Dexterity, or Constitution. Curing requires killing the mote colony, but does not restore lost attributes.
Thinblood
Gashes and pinpricks alike spill forth unclotting. Caused by bites from blood-sucking bats. Every combat round (or minute), take 1d4 damage from blood loss per physical wound. Sufficient bandaging stops the bleeding, but not the affliction. Lasts 1d4 days.
Marbleye
An iris crystalizes white and the pupil grows milky until it's a mere pit in the sclera. Transmitted by some water leeches or getting dangerous water in the eyes. When submerged in the stagnant waters of the Steaming Marsh, Save vs Poison at +3 or contract marbleye: one eye grows dim over 1d10 days. Every year, Save vs Poison or the eye goes blind. Acclimation to one-eyedness takes months. If afflicted again, the other eye contracts it. No known cure.
Stillmind
Intellect is ground to a blunt corner: learning slicks off it. Inflicted by certain drugs and infohazards. Save vs Will or lose 1d6 Intelligence and 1d6 Wisdom. Additionally, the stillminded learn and gain XP at half the normal rate. Every week, Save vs Will to recover.
Netherly Haze
Stokes the libido and gnaws the mind into a brainfever. Sexually transmitted via genitals, no save. When infected, lose 1d4 Wisdom from a mental fog and itching libido. Every month without intercourse, Save vs Will or lose 1 Wisdom. No known cure, but removal of sex drivers stops the Will Saves from triggering.
